FORM SUBMISSION

Welcome to Form Submission

Form Submission is an interactive web-art piece, which uses a network of online forms and automated emails to explore the unique ways that digital bureaucracy not only serves as a Neoliberal control mechanism, but also renders its users abject through the continual quantification and (re)adjustment of their identity. To learn more about it, please visit the About section.

IMPORTANT DISCLAIMER ONE

This work asks for personal information, which is stored on a database on this site. While the database is secure, the protections in place are not equivalent to that of a bank, state or other high security network, and the absolute privacy of your information cannot be guaranteed. You will not be asked for any financial information, your Social Insurance / Social Security number, or your Postal / Zip code, but the information requested will include potentially sensitive personal details. While the work is most impactful when used with your actual information, if you feel at all uncomfortable sharing something you are encouraged to use false information as needed. The only thing you absolutely need to input accurately to interact with the work is a functional email address

Your information will not be shared in any way, or used for any purpose other than this work

Upon reaching the final page of the work any personal information on the database will be permanently erased. If you do not view the entire work, please go to the Reset Page where you can enter the email you signed up with to wipe all personal information from the database. You can access the Reset Page at any time using the menu on the top right corner of the page.

You can also use the Reset Page if you reach a point where you can no longer progress within the piece. There are several points where this might happen, especially if you navigate away from any of the pages. Wiping your personal information will allow you to start over from the beginning.

IMPORTANT DISCLAIMER TWO

This work modifies the personal information which is submitted to it, in order to explore the ways that identities are manipulated or made abject by digital bureaucracies. As a result, you may be misnamed, misgendered, or otherwise misidentified in the course of interacting with this work. If you are especially troubled by being misidentified in this way, please proceed with caution or consider whether or not you wish to continue to the work. While this element of misidentification and its role within Neoliberal structures is a part of the work, the goal is to explore and critique violence of this kind, not to repeat it.
